Support At Home Clinical Care Partner Lumia Care is seeking an experienced and compassionate Clinical Care Partner to join our in-home aged care team. About the Role As a Clinical Care Partner, you will operate autonomously while collaborating with a dedicated team. You will coordinate personalised in-home care for participants funded under the Australian Government's Support at Home program. This role combines clinical expertise, exceptional communication, and a participant-centred approach to deliver high-quality care and support. Key Responsibilities: Conduct home visits, assessments, and develop individualised care plans Build trusted relationships with participants, families, and health teams Lead quality care delivery while supporting operational and clinical goals Manage complaints, feedback, and ensure compliance with aged care standards Collaborate with the team to support continuous improvement and accreditation What You Bring: Registered or Enrolled Nurse with 3 years’ experience and current AHPRA registration Strong clinical skills and knowledge of aged care legislation and in-home programs Excellent communication, interpersonal, and stakeholder management skills NSW Driver’s License and comfortable using IT systems and portals Passion for delivering participant-centred, high-quality care Why Join Lumia Care?
