Company/ Role: We are looking for a motivated and compassionate Community Physiotherapist to join our dynamic Physiotherapy team. You will provide high-quality physiotherapy assessment and treatment to patients in their own homes, care homes, and community settings — helping people regain independence and improve quality of life. Areas that we service include Sydney's Inner West & Eastern Suburbs. Key Responsibilities Deliver evidence-based physiotherapy interventions in a variety of community settings. Assess, plan, implement, and evaluate individual treatment programmes. Work collaboratively with occupational therapists, nurses, social workers, and other healthcare professionals. Support hospital discharge and prevent avoidable admissions by promoting safe mobility and functional independence. Maintain accurate and timely clinical documentation. Contribute to service development and quality improvement initiatives. About You AHPRA-registered Physiotherapist (BSc or MSc in Physiotherapy). Experience in community, rehabilitation, neuro or elderly care settings desirable. Strong clinical reasoning, communication, and organisational skills.
