The role also involves implementing the Territory Kidney Care (TKC) initiative in partnership with key health organisations - embedding renal care planning and risk reduction strategies into primary health care to improve long-term outcomes for people with chronic conditions. About You Minimum 5 years’ experience in chronic disease management and/or regional health program delivery. Proven clinical leadership or supervisory experience, ideally in a remote Aboriginal health context. AHPRA registration as a Registered Nurse, Allied Health Professional, or equivalent.
