About the role South West Healthcare (SWH) is seeking applications from suitably qualified Registered Nurses or Allied Health Professionals for the position of Transition Care Coordinator. The Transition Care Program provides short-term, goal-oriented support to older clients following a hospital stay, helping them regain independence either at home or in a residential setting. As the Care Coordinator, you will provide care planning, coordination, and support to clients and their carers, acting as the central point of contact throughout their transition. You'll work collaboratively with multidisciplinary teams, internal departments, and local health services to ensure seamless, person-centred care and positive outcomes. Position overview Employment Type: Casual - providing leave coverage and additional support during periods of increased demand Location: Warrnambool Campus Classification: Liaison Officer District Nursing, or Allied Health Professionals Grade 3 What you bring You will hold a relevant Allied Health or Nursing qualification with current registration and demonstrate a strong commitment to ongoing professional development and maintaining required credentialing standards. Desirable qualities include: Sound understanding of aged care, the Transition Care Program, and the Care Coordinator role within this service. Clinical experience in care coordination, case management, or discharge planning, particularly within aged care or rehabilitation settings. Familiarity with Aged Care Assessment Services and relevant Aged Care legislation relating to residential placement. Proven ability to work collaboratively within an interdisciplinary team, building and maintaining effective working relationships.
