The role of the Enrolled Nurse is responsible for providing direct nursing care. Working under supervision, the Enrolled Nurse provides nursing care to ensure the delivery of services meets the required aged care standards and supports the wellness and reablement philosophy. About The Role Some of your responsibilities will be: Provide direct nursing care to clients of the Home Care Division in line with evidence-based practice (EBP) and current guidelines, including holistic assessment of a client’s health.Using the appropriate assessment tools conduct comprehensive assessments, reassessments and evaluations of client care needs, reporting any changes in the client’s condition to the relevant clinician or referrer. and updating relevant documentationDevelop, facilitate implementation and evaluate care management plans for clients with complex health needs in collaboration with RN or relevant clinicianDetermine clients’ support requirements and work with the team to establish services and arrange referrals to other Health Professionals as requiredConducting client health assessments and providing recommendations under the Specialised Support Services program, including but not limited to continence and dementia advisory services.Participate in professional supervision, staff mentoring, staff education, training and development, staff communication and performance reviews About You To be a successful candidate you will have: Ability to undertake the duties of the role in accordance with CatholicCare’s Mission, Vision and ValuesRelevant tertiary qualifications in Nursing (such as a Diploma of Nursing) and current registration with the Australian Health Practitioner Regulation Agency along with demonstrated experience in an EN role preferably in a community setting with an understanding of the aged care sectorKnowledge of community supports available for clients and an ability to negotiate with internal and external stakeholders to enhance the quality and level of service to clients.Ability to work as part of a team, engage with people from all socio-economic and cultural backgrounds to meet organisational and client outcomesSkills and knowledge in safe manual handling procedures, documentation skills, medication management, infection control, palliative care and wound managementExperience responding to issues identified by clients in a resource efficient and timely manner and using problem solving skills to find a resolutionDemonstrated oral, written communication and presentation skills across a broad range of disciplinesDemonstrated capacity to work effectively with First Nations and CALD colleagues and clientsCurrent Driver’s Licence and access to a well maintained registered vehicleNational Criminal Record History Check If you would like to find out more about the role, click apply to view the Position Description on our career website.
