Offer education and practical guidance to clients, families, and carers to support improved nutrition and wellbeing. Collaborate with GPs, nurses, and allied health professionals as part of a multidisciplinary care team. Assist clients with managing conditions such as diabetes, cardiovascular disease, weight management, and malnutrition. Travel to client homes within the community (car allowance provided).
