About the Role: In this key position, you will coordinate and oversee the delivery of safe, high-quality, and person-centred care for our aged care and disability clients. You will be responsible for maintaining a caseload, performing clinical assessments, and ensuring care aligns with client needs, care plans, and available funding. Key Responsibilities: Conduct comprehensive client assessments and develop care plans Maintain compliance and manage a caseload of clients Coordinate care and services within budget and funding allocations Perform administrative duties related to client care coordination Liaise with clients, families, and internal/external stakeholders What We’re Looking For: Diploma of Nursing and current AHPRA registration (Enrolled Nurse) Prior experience in aged care, disability, or mental health care coordination Knowledge of My Aged Care, Home Care Packages, and/or NDIS (desirable) Strong assessment and care planning skills Excellent communication and customer service skills Ability to manage tasks independently and within a team Proficiency in prioritising, planning, and problem-solving Confidence in managing phone-based and face-to-face communication professionally Strong networking ability and comfort in engaging with external organisations Commitment to confidentiality, compliance, and organisational values Why Join Us?
