About the company: Hills District Home Care Pty Ltd, operating as Home Caring Hills District, is an approved NDIS and Home Care Package (HCP) provider who provides disability support and related services to seniors and individuals with disabilities in the Hills District, NSW region. We are looking for a Residential Care Officer who could support our clients living in SIL/SDA accommodation settings - in enhancing their daily living skills, fostering social engagement, and promoting emotional well-being. Overall, this role will enable the participating clients in their goal attainment, greater independent living and community participation. Location: STANHOPE GARDENS NSW, 2768 Salary range: $70,000 - $90,000 per annum (depending upon the experience) Role: Full-time Positions: Multiple Key responsibilities include: Develop, implement, and monitor individualized care plans for clients to enhance their independence. Support clients with their mental health, social and emotional support needs and behavioural support plans to promote independent living Supervise daily routines ensuring personalized support to clients in shared living environment to enhance their independence and skill development. Conduct interviews with clients to assess the nature and severity of issues Ensure clients get connected with their family, community, and culture. Facilitate participation of clients in community activities and social engagements to promote inclusion and well-being. Monitor and report client progress and perform incident reporting Implement positive behaviour support strategies helping clients lead more fulfilling lives in alignment with their personalized care goals Ensure a safe and comfortable home like environment, addressing any potential hazards and promoting therapeutic practices. Refer clients to agencies for additional support. Manage budgets and funding for clients for cost effectiveness. Assess client’s need and provide support for health, welfare, employment, housing, training, and other appropriate services for the wellbeing. Assure high levels of support by liaising with welfare agencies. Provide guidance and mentoring to junior support staff Relevant Qualifications and skills: Minimum Cert III or IV in Individual Support, Community Services or Diploma in community services or equivalent qualification in a related field. In absence, of relevant qualification, three years of relevant experience may substitute for the formal qualifications. Minimum 2 years of related experience preferably in disability/mental health setting/aged care in Australia Experience with high needs or critical care clients (desirable). Flexibility to travel across different locations in the Hills District based on client needs. A solid understanding of NDIS and disabilities in Australia Good customer service skills Exceptional time management skills Administrative /Case Management skills Excellent communication and writing skills.
