About the role and where you will be working: Our Care Coordinators coordinate, guide & oversee service provision (and information) to our Home Care customers. You'll be helping them make decisions, identify and arrange resources that meet their identified care needs. We'll also be asking you to oversee day to day management of funds / budgets. This is a mobile and office-based role, with the majority of your time spent in the community visiting clients in your care portfolio. We’ll also be asking you to: Providing great customer service & advocating for your clients Undertaking comprehensive assessments of identified needs Developing goal & care plans provide guidance to promote enablement through informed decision making and workable solutions Completing all required documentation Identifying & responding to customer's changing needs Working collaboratively with clinical & allied health services About you: With a well-developed knowledge of the aged care sector, you have experience in a coordination / case management role. As a natural problem solver, you can work autonomously and also work effectively as part of a multidisciplinary team. You're comfortable managing complex clients who are receiving Home Care packages. To be considered for this role, you’ll need: Experience in aged care or community services. Familiarity with home care packages. Great interpersonal & communication skills.
