About the Role The Clinical Care Partner is responsible for delivering care management activities that enable older people to live safely and independently at home. Working within a rights based, person centred framework the Clinical Care partner collaborates with consumers, families and interdisciplinary teams to coordinate tailored, responsive and inclusive support. Clinical Care Partners function as relationship-based care coordinators who provide ongoing planning, monitoring, service coordination and education – with a strong focus on wellness, reablement and culturally safe care. Key responsibilities include: Conduct initial assessments for new consumers joining the LHG Home Care Service Identify required services and develop person centred support plans for new consumers, ensuring alignment with the Notice of Decision, formal aged care assessments and approved Support Plan. Complete a formal review of the care/support plan, involving all key stakeholders, as directed by program guidelines or as change is indicated. Provide skilled and timely care to consumers whilst promoting their independence and involvement in care decision–making. Ensure services are consumer focused and based on dignity, respect, cultural sensitivity and informed choice Promptly follow up on referrals received from various sources Demonstrate commitment to continued learning and participate in training and development activities. About You Bachelor of Nursing with current AHPRA registration. Previous experience working in a coordination capacity for a Home and Community aged care organisation will be highly regarded. The ability to work autonomously, self-motivate and operate with minimal supervision is a necessity for success in the role. A strong theoretical approach to working with ageing population and demonstrated practical application of this approach. Candidates may be subject to a pre-employment medical assessment with the successful candidate requiring current NDIS worker clearance.
