Some of your responsibilities in this role will include: Conducting assessments and managing intake in line with agency policies Providing tailored, client-focused case management Informing clients of their rights and responsibilities and encourage active decision-making Advocate on the behalf of clients regarding housing and related issues, and connect them with a range of support services to meet their individual needs. Your skills and experience Current drivers licence valid in Victoria Qualification in Human Services field e.g. Social, Work, Youth Work, Welfare studies OR where you can demonstrate relevant work experience and/or studies towards completing one of the above qualifications Demonstrated experience in direct service case management Prior experience with use of client data entry program SHIP, preferable but not essential Demonstrate a sound knowledge of casual factors associated with homelessness and the needs of homeless families/individuals Demonstrate capacity to work with homeless families and individuals who present with complex needs and trauma.
