(pro-rata) Superannuation & Salary Sacrifice Benefits Adelaide SA, 5000 Part Time, Contracted Role working 22.5 hours per week up to 29 Sep 2028 About the role This is an excellent opportunity for a senior pharmacist to join the CALHN Integrated Care team. Integrated Care provides services uniquely at the interface of hospital and community, promoting ‘home first’ care, hospital avoidance and continuity of care. Integrated Care encompasses a range of programs, including, but not limited to Urgent Care/Hospital avoidance hubs, Hospital in the Home and community geriatrics programs. This role will have an initial focus supporting implementation of the CALHN Geriatric Outreach Service (GOS). GOS is a multidisciplinary team working with consumers experiencing significantly changed behaviours relating to dementia and/or cognitive decline, in the setting of residential aged care. The program supports transition from hospital to community and keeping older consumers in place within residential aged care (hospital avoidance) through timely intervention and support for the older person, their family and residential aged care facility (RACF) staff. The pharmacist will support medication history and identification of contributing factors and communication of medications changes for continuity of care. They will also support education for consumers, their families, RACF staff and CALHN staff. This is predominantly a clinical pharmacy role, able to be shaped by the incumbent with opportunities for research and quality improvement. Applicants with experience in complex medication management in older persons, including those living with dementia, and those with experience undertaking community-based medication reviews, are encouraged to apply. More broadly you will be part of CALHN Pharmacy with access to the full range of expertise available in our dispensing, electronic medication management, manufacturing, investigational drugs, clinical, and medication governance teams. Some Integrated Care services operate across 7 days, with access to the multidisciplinary team critical to the success of the model. Applicants should expect to be rostered on weekends at a frequency of up to 1 weekend per fortnight, although the role will initially be rostered Monday to Friday. About You You are a motivated and detail-oriented Pharmacist with a strong foundation in clinical practice and a passion for improving patient outcomes. With excellent communication and leadership skills, you are confident in managing complex workloads, supporting colleagues, and driving service excellence in a hospital pharmacy environment. Registered (or eligible) with the Pharmacy Board of Australia, holding a Bachelor of Pharmacy (or equivalent), with extensive experience in hospital pharmacy and/or geriatric medicine practice. Strong communication and interpersonal skills, with the ability to build positive relationships, educate staff, and work effectively within a multidisciplinary team. Proven organisational and problem-solving abilities, with the capability to research, analyse data, and implement strategies for improving pharmacy services. Comprehensive knowledge of pharmacy practice, codes of practice, and professional standards, with demonstrated leadership, supervisory, or management experience.
