We are seeking a Clinical Care Partner to join our Support at Home team and make a real difference in the lives of our clients. About the role As a Clinical Care Partner, you will deliver high-quality, person-centred, culturally safe clinical care management services to older people living at home. Working in alignment with the Support at Home program and our clinical governance framework, you will play a vital role in ensuring clients maintain independence, wellbeing, dignity, and safety in ways that reflect their culture, identity, and preferences. You will: Undertake comprehensive clinical assessments, care planning, and service coordination. Provide evidence-based clinical risk assessment, education, and guidance for clients with complex needs and chronic health conditions. Offer clinical oversight and support to non-clinical Care Partners, ensuring early identification and management of clinical risks and deterioration. Ensure services are delivered in line with the Strengthened Aged Care Quality Standards. About you You are a dedicated and compassionate health professional who thrives on building meaningful connections with clients and colleagues. With a strong clinical background, you are confident in providing oversight and coaching, while ensuring the highest standards of care. You will bring: Current registration with AHPRA as a Registered Nurse (or equivalent clinical qualification). Experience in community care, aged care, or chronic disease management. Strong clinical assessment and care planning skills. The ability to work collaboratively across multidisciplinary teams. A commitment to person-centred, culturally safe care.
