The Home Care Manager is responsible for managing and coordinating home care services provided to elderly individuals in accordance with Hope Holistic Care's (HHC) values, policies, and the requirements of relevant government regulations. This role involves overseeing a small team of nurses (RN), Care Partners and care helpers, developing care plans, managing budgets, and ensuring that high-quality care is delivered to support the well-being and dignity of clients. The manager will also foster relationships with clients, their families, staff, volunteers and the broader community, ensuring that services are compassionate and aligned with HHC's mission. ___ Key Responsibilities: Service Management: Oversee the daily operations of home care services, ensuring that care delivery meets both internal quality standards and regulatory requirements. Develop and implement individual care plans tailored to the needs of each client, ensuring their physical, psychological, social and spiritual well-being. Ensure compliance with relevant aged care standards, funding requirements, and government regulations. Collaborate with clinical staff and healthcare professionals to assess and monitor clients' needs. Manage client intake and assessment processes, ensuring appropriate allocation of services. Team Leadership: Lead, supervise and support a team of care partners and helpers Provide training and development opportunities to enhance staff skills and maintain high-quality care. Ensure that staff work in line with the organization's mission, values, and ethical guidelines. Work with rostering team to ensure staffing is adequate for service coverage. Client & Family Engagement: Act as a liaison between clients, families, and the care team, ensuring open communication and responsiveness to client needs and concerns. Promote the spiritual care of clients in accordance with HHC's values. Address and/or escalate any complaints or grievances promptly, aiming for resolutions that are compassionate and aligned with HHC's values. Financial & Operational Management: Under the guidance of COO and CFO, develop and manage the home care services budget, ensuring efficient use of resources. Monitor funding agreements, subsidies, and billing to ensure services are financially sustainable. Explore opportunities for service growth and expansion in the community. Quality & Compliance: Ensure continuous improvement of services through regular audits, reviews, and feedback mechanisms. Stay informed about changes in aged care legislation and best practices, implementing necessary changes to service delivery. Maintain and update policies and procedures relevant to the role Ensure that health and safety protocols are followed to protect staff and clients. __ Qualifications: Bachelor's degree in Social Work, Nursing, Health Management, or related field (or equivalent experience). Previous experience in aged care or home care services, preferably in a leadership role. Strong understanding of (or working towards) the aged care system, including relevant legislation, funding, and service standards. Excellent interpersonal and communication skills, with the ability to build relationships and manage teams effectively. A commitment to HHC's values to provide physical, emotional, psychosocial and spiritual care to clients. Ability to work with budgets, analyse financial performance and optimise operational efficiency. Experience in dealing with sensitive issues, such as end-of-life care, in a compassionate manner. Have a NSW Drivers License and is able to travel within metropolitan Sydney as required. Fluency in Cantonese and/or Mandarin , with written and verbal communication skills in both Chinese and English. Current NDIS Worker Screening Check (or ability to obtain), First Aid certification, and satisfactory National Police Check. Must have unrestricted rights to work in Australia (Australian citizenship, permanent residency, or appropriate work visa). Desirable: Experience working in a non-profit or faith-based organization. Knowledge of specific faith traditions and spiritual practices that support the well-being of elderly individuals. Certification or training in palliative care or gerontology.
