About the role Are you passionate about supporting vulnerable young people and helping them build brighter futures? We are seeking a dedicated and experienced Youth Case Manager to join our team in Chisholm, ACT, on a full-time 12-month max term contract . This role provides safe and secure accommodation and professional support for young people experiencing homelessness. You will work within a person-centred framework to deliver flexible, individualised support and case management that promotes independence and community connection. How you will make an impact Develop and maintain person-centred case management plans tailored to individual needs Provide holistic support addressing physical, emotional, mental, and social wellbeing Conduct risk assessments and develop comprehensive safety plans Regularly review case plans and progress Maintain accurate service documentation and data Support young people in building essential life skills for independent living Collaborate with the Life & Living Skills Worker to develop and deliver relevant training Facilitate access to internal and external services and advocate on behalf of young people Contribute actively to a supportive and inclusive team environment Build strong professional relationships with external agencies and within the TSA network Engage in continuous improvement and team culture initiatives Ensure safety practices align with organisational policies and WHS legislation What you will bring Diploma-level qualification (or higher) in Youth Work, Community Services, or a related field Experience working with young people in homelessness or community services Strong understanding of trauma-informed care, mental health, and substance use issues Proven ability to work collaboratively with external stakeholders and services Excellent documentation and case note writing skills ACT Working with Vulnerable People card Current ACT Driver Licence Senior First Aid Certification What we offer The Salvos offer eligible employee's a well-balanced package of meaningful benefits including: Salary packaging up to $15,900 tax free $2,650 meal entertainment benefit.
