As this role does support our regional sites frequent travel to the region maybe required. If you are a Nurse looking for a change this maybe an opportunity for you. Some of your duties may include: Active involvement in admission process including but not limited to pre-screening, AN-ACC funding predictions, commencement of assessment and care plans and communication with the clinical team Assist with the monitoring and tracking of residents’ data including all movements, Medicare events and AN-ACC lodgements Review assessments to care plan to ensure congruency and ensure it is reflective of residents needs to support AN-ACC Classification. This includes but is not limited to pre-admission screening, new admission screening and reclassification screening Deliver high quality, effective documentation, reporting and tracking. Monitor Occupancy, Entry and Departures Ensure each AN-ACC assessment has detailed explanation to support the scoring in the screening document Complete allocated KPI’s for resident screening, reclassification requests each week in order to support sites KPI for PRPD Spend time with carers and the clinical team on a daily basis across shifts to assist and support with documentation and reporting Actively participate in monitoring resident conditions, clinical meetings and feedback from clinical teams, handovers and head of department meetings to gather information around potential reclassification opportunities What will you bring to the role? Experience in a care or clinical role within a Residential Aged Care Facility Knowledge and understanding of the AN-ACC tools, AN-AC funding guide and processes in a Residential Aged Care Facility Strong written and communication skills Proven ability to quickly acquire knowledge of diverse software systems together with a working knowledge of the Microsoft suite of products Demonstrated effective time management skills Ability to work as part of a team and with minimal supervision Attention to detail and great customer service skills and ethic Ability to travel to sites when required Who is Southern Cross Care Queensland?
