The Client Major Force Australia is a disability support and health care services company based in Upper Coomera in QLD. They are looking a full time Community Worker at Upper Coomera in QLD. The Role This is for a full time role with an annual salary of $77,000 to $80,000 depending on experience plus superannuation and will be based at Upper Coomera in QLD. Duties will include: Study the care plan and implement the support, education and training program in place Observe, monitor and record clients' progress through case notes for every shift Attend meetings when required, participating in discussions whenever an assessment is required to review the client's community needs and resources for health, welfare, housing, training and other facilities and services Provide support in clients' houses and provide education and care for disabled persons in adult service units, group housing and government institutions Support clients to overcome social, emotional, and financial problems Assist in preparing reports for necessary submissions to government entities or other stakeholders for funding and resources and reports to government bodies and other agencies Supporting clients on community outings, e.g., shopping trips, excursions, lunches, appointments, and goal-based activities Escorting clients to medical and allied health appointments, hospital visits, and respite care Cleaning, tidying, and changing bedding to ensure a safe, clean, and sanitized environment for clients Requirements Diploma in Community Services or similar qualification. Minimum 2 years of relevant experience in a professional setting Minimum 2 years of relevant experience in community or aged care Minimum 2 years of experience dealing with mental and emotional disturbances and developmental disability Minimum 2 years of experience working with government agencies, non-profits, and other service providers Have a current or be willing to obtain a First Aid (HLTAID003 or HLTAID004) and CPR (HLTAID001) certificate Excellent communication and interpersonal skills
