St Vincent’s Hospital, Lismore is looking for a qualified social worker to provide a contracted service. The provision of a high level of tailored support services to our patients, their families and carers, will be through a referral system. The Social Worker within St Vincent’s Hospital will be required to provide assessments, counselling, mediation and case management to patients within the hospital setting. This position will also be required to liaise with the multidisciplinary team. This will ensure a high level of person centred care when discharging; providing invaluable knowledge and support to families and carers of our patient’s.
