The Role We are seeking a compassionate and motivated Disability Services Officer to join our growing team. In this role, you will provide direct support to clients, coordinate individual care plans, and foster community inclusion. You will work closely with participants, families, and support networks to ensure each client receives personalised, holistic care. Key Responsibilities Support clients with daily living tasks, personal care, and community participation. Develop and implement individualised support plans in line with NDIS requirements. Assist clients in achieving their personal, social, and employment goals. Promote inclusion, independence, and dignity in all aspects of service delivery. Maintain accurate records, progress notes, and compliance with organisational policies. Liaise with families, healthcare professionals, and community organisations. Skills & Experience Qualification in Disability, Community Services, Aged Care, or related field (Certificate III/IV or above preferred). Experience working in disability support or community services. Strong communication, interpersonal, and problem-solving skills. Knowledge of NDIS frameworks, rights of people with disabilities, and person-centred care approaches. Ability to work independently and as part of a multidisciplinary team. Current First Aid & CPR Certificate, Blue Card, Yellow Card (or NDIS Worker Screening Check). Valid driver’s licence and reliable vehicle.
