They foster a supportive environment where both clients and staff can thrive, and they’re now seeking an experienced Intake & Engagement Officer to join their team based in Cannon Hill. About the Role and your Responsibilities: Reporting to the Office Manager, the Intake & Engagement Officer will coordinate the intake process for new participants, guiding them from initial enquiry through to the start of services with a smooth and supportive experience. This role is central to building trusted relationships with clients, veterans, community organisations, and allied health professionals through clear communication and responsive service delivery. By nurturing referral networks and ensuring person-centred engagement, you’ll contribute to sustainable participant growth and a positive introduction to services. Some of your responsibilities will include: Manage the full participant intake journey, from first enquiry to service commencement, ensuring a smooth, supportive, and efficient experience. Conduct initial engagement and sign-up meetings to assess client needs and explain available care options (Aged Care, Home Care Packages, Disability/NDIS, DVA Community Nursing). Complete data entry, service coordination, and scheduling/rostering of initial services following sign-up. Maintain accurate and up-to-date records of referrals, communications, and care decisions within the client management system. Collaborate with the Care Management team, Office Manager, and leadership to support participant growth and track KPIs. Participate in the on-call roster to ensure consistent support for clients and staff. Build and maintain strong referral networks with community groups, allied health professionals, hospitals, LACs, plan managers, and other stakeholders. Act as a professional, trustworthy representative of the organisation when engaging with clients, partners, and the community. Prepare monthly reports on intake metrics, engagement outcomes, and partnership activities for the Office Manager and Management Committee. Support wider office functions when required, including assisting staff with information and resources to deliver excellent client care.
