About the role We are seeking a compassionate and community-minded individual to join our Partner in the Community (PITC) team as a Pathway Coordinator – Lived Experience. This unique role draws on your personal experience of living with disability to support others navigating the National Disability Insurance Scheme (NDIS) and related services. As a PITC Pathway Coordinator, you’ll be the first point of contact for individuals with disability, providing guidance, and support throughout their journey. You’ll work collaboratively across regions to ensure person-centered, inclusive, and accessible service delivery. Your lived experience will be central to embedding the consumer voice in everyday practice. Key Duties · Provide guidance, support and advice to customers and assist them with navigating the National Disability Insurance Scheme (NDIS) Access Pathway and NDIS Participant plans · Support customers to complete and submit documents that meet requirements for Scheme Access Requests · Refer individuals to the appropriate Local Area Coordinator (LAC) or Early Childhood (EC) Teams once the necessary documentation has been provided · Maintain asset maps and ensure community connections are in place for individuals aged 9 years and over · Build Community Connections Plans as required · Work collaboratively across the multi-disciplinary EC teams to ensure the entire Early Childhood pathway is managed smoothly for children under 9 years of age · Utilise the Towards Inclusion Framework (TIF) to build individual and family capacity to clarify their goals, exercise choice and control and to participate in the community · Ensure the timely provision of information, and linkages to mainstream services for participants and non-participants · Respond to all incoming calls and general inquiries that come through via the 1800 number and emails sent to the Baptcare team inboxes · Support customers to complete and submit documents that meet requirements for Scheme Access Requests · Follow all confidentiality, privacy and conflict of interest policies and procedures · Consistently work within relevant standards and legislation including but not limited to: The NDIS Act 2013 and associated rules Children, Youth and Families Act 2005 (Vic) Children, Young Persons, and Their Families Act 2009 (Tas) Child Safe Standards (Vic, but applicable to Tas) Carer Recognition Act 2010 (Cwlth) and 2012 (Vic) Children and Young People (Safety) Act 2017 (SA) · Provide information and support on the NDIS to service providers, people with disability, their families, and the community · Contribute to a positive team environment · Undertake other relevant duties as required Workplace Health & Safety · Report WHS hazards, injuries, and incidents (including near misses) · Act responsibly to protect your safety and the safety of others · Participate in and contribute to WHS improvement activities including inspections, audits, and other review activities · Ensure tools, equipment, PPE, and clothing are in a safe condition before use and report/fix any defects as appropriate · Actively participate in rehabilitation and return to work activities following any work-related injury · Comply with responsibilities under the WHS Management System and any reasonable instructions, including during emergency situations · Ensure any incidents you encounter are reported, supporting any investigations and the implementation of preventative measures About You You bring empathy, insight, and a commitment to inclusion, along with the following: Lived experience of disability (highly desirable). A relevant qualification in Disability or Community Services, or 2 years’ experience in the sector. Strong communication and interpersonal skills. Ability to work independently and collaboratively. Proficiency in Microsoft Office and client record systems. Understanding of relevant legislation and child safety standards.
