The St Vincent’s Older Adult Mental Health Service comprises community care via the Aged Psychiatry Assessment and Treatment Team (APATT), inpatient care via Normanby Unit (20 bed acute mental health inpatient care) continuing care via the Psychogeriatric Nursing Homes (PGNHs) and early intervention via the Healthy Ageing Service (HAS). You will be visiting clients at home, providing intake and triage, crisis support, assessment and case management, reviewing risk and providing support to reduce impact of illness and enhance independence to our clients and carers in a variety of community and residential settings. You will provide discipline specific support and advice to the team. We utilise a cooperative and recovery-focused approach across the continuum of care to achieve optimal outcomes for consumers. Classifications for this position: Registered Psychiatric Nurse Grade 4 NP75 - NP77 ($112,465 - $121,113 per annum pro rata ) Your Contribution Relevant postgraduate qualifications Current registration with the Australian Health Practitioner Regulation Agency (AHPRA) 5 years post qualification clinical experience within Mental Health, care of older adults with clinical services experience including case management and care coordination Demonstrated passion for achieving the best clinical practice Relevant knowledge of the Victorian Mental Health and Wellbeing Act 2022 and other relevant legislation What we Offer Salary Packaging – Increase your take home pay!
