About the Role Your role as the Community Access Facilitator – Specialist Disability Accommodation you will oversee, develop and implement community access experiences that are the choice of residents, either internally or externally. These activities must be aligned with the individual community access goals and be within each resident’s community access budget. These activities must provide choice, flexibility and spontaneity. Key Role Accountabilities: Oversee the development of NDIS Community Access programs and plans for each resident. Liaise with residents and primary support networks to develop and implement and review NDIS Community Access plans monthly. Develop, submit and oversee NDIS Community Access Service Agreements. Document and report ongoing progress and budget of resident’s Community Access programs. Complete risk assessments for all new community access activities for individual and groups of residents and review existing risk assessments annually. Engage, train and oversee personal carers and volunteers to provide a high quality community access experience either in house or in the community. Assist with Activities of Daily Living as required by residents / clients within scope of training and qualifications. Access and support volunteers and link them to the needs and interests of residents. Liaise and engage with Support Coordinators in the development of the Community Access Program for each resident. Liaise and engage external organisations/providers to meet the individual needs of residents, so they can attend mainstream community access activities, with appropriate supports either internally or externally. About You Demonstrated experience working with people with a disability. Demonstrated experience in the planning, implementation, facilitation and ongoing review of innovative and flexible recreational programs. Ability to lead a small group of support workers and volunteers to deliver community access programs and activities. High level of verbal and written communication skills and experience developing professional rapport with clients. Highly effective interpersonal, and communication skills including the ability to develop rapport and maintain effective relationships with people who have a disability, their families and networks. Demonstrated ability to deliver support in a way that assists a person who has a disability to attain and maintain their optimum degree of independence and quality of life and to pursue their goals. Demonstrated understanding and commitment to the principles and practices of privacy, confidentiality and professional boundaries in relation to all customers and staff. What makes a difference for us Qualifications in Leisure and Lifestyle Certificate IV or other relevant specialist area. Other specialised qualifications in areas of disability, music, art, aromatherapy or other skills related to the work of Leisure and Lifestyle will be highly regarded but will not change the Level 3 classification for the scope of work required for this role. Proof of Vaccination Status as required by Legislation, Health Directives or WMQ Policy throughout employment and prior to commencement. Relevant probity checks required by legislation and WMQ policy. Current Driver’s Licence and willingness to drive in the course of work. Why WMQ?
