About the role: OOHC Case Manager - Change Lives, One Young Person at a Time Are you ready to make a real difference? In this pivotal role, you’ll work with children and young people aged 12-24 who are at risk or experiencing homelessness, helping them heal, grow, and thrive. No two days are the same; you’ll be a trusted mentor, advocate, and problem-solver, creating safe and supportive environments where resilience is built, connections are forged, and futures are reshaped. From guiding emotional regulation and facilitating life skills programs, to managing individualised, trauma-informed case plans, you’ll play a hands-on role in empowering young people to overcome challenges and become the very best version of themselves. Duties include: Managing a caseload of up to 6 young people where you will facilitate and organise case conferences. Build, apply and review case management plans and exit plans. Be a positive role model and provide advocacy for young people as needed. Identify triggers and risks and respond in accordance with best therapeutic practice. Respond to conditions that create “crisis” and require therapeutic management. Implement a variety of strategies and coping mechanisms for the young people to learn from and meet their needs. About you: Degree in relevant Social Sciences (e.g. Social Work, Psychology, etc). Further relevant qualifications would be desirable.
