The Client Everlasting Care Disability Care Pty Ltd is a registered NDIS provider based in Lansvale, NSW. The Role This Community Worker role is for a full time role with an annual salary of $77,000 depending on experience plus superannuation and will be based at Lansvale in NSW. Duties will include: Assess clients' needs and plan, develop and implement educational, training and support programs Interview clients and assess the nature and extent of their difficulties Monitor and report on the progress of clients through case notes and review meetings Refer clients to relevant agencies and community-based organisations that can offer additional help Assess community needs and available resources for health, welfare, housing, employment, training, and other services Liaise with community groups, welfare agencies, government bodies, and private entities to address community issues and promote awareness of available resources Support families and provide education and care to children and individuals with disabilities in residential and institutional settings Supervise offenders on probation and parole, ensuring compliance and social reintegration Assist young people in resolving social, emotional, and financial challenges Prepare funding submissions, reports for government bodies, and updates for stakeholders and support coordinators Deliver support services based on individualised NDIS goals, including community access and daily living support Support clients in participating in social, educational, recreational, and goal-based activities Provide personal care, assist with domestic duties, and administer or prompt medication where needed Escort clients to health appointments, shopping trips, events, and respite care Foster respectful, empowering relationships with clients, families, and care teams Collaborate with allied health professionals and service providers to ensure coordinated care Maintain accurate documentation including shift notes, incident reports, and client progress logs Follow Work Health & Safety (WHS) procedures and respond appropriately to client concerns Requirements Diploma of Community Services (or equivalent) Minimum 12 months experience supporting individuals with disability or in community services Minimum 12 months of relevant experience in community or aged care Minimum 12 months of experience dealing with mental and emotional disturbances and developmental disability Demonstrated understanding of NDIS and its core principles Strong experience in person-centred care and planning Valid First Aid (HLTAID011) and CPR (HLTAID009) certification Australian Driver's Licence and reliable, insured vehicle NDIS Worker Screening Check and Working with Children Check (or willingness to obtain) Excellent communication, documentation, and interpersonal skills Ability to work independently and within a multidisciplinary team
