Description Join Our Team at Right at Home Ballina: Home Care Coordinator Opportunity Right at Home Ballina is seeking an experienced and motivated individual to join our dynamic Home Care team as a Care Coordinator . With the rollout of reforms under the Support at Home program, we're urgently looking for a proactive, compassionate, and forward-thinking Care Partner to help lead our service delivery. This is a full-time opportunity for someone who thrives in a fast-paced environment, excels at coordinating care services, and is passionate about driving growth while supporting Home Care Package (HCP) recipients. This role goes beyond traditional case management—it’s leadership in action. About the Role In this vital position, you’ll oversee the coordination of services under the Support at Home framework—including nursing, allied health, home support, and equipment—empowering clients to make informed decisions and exercise choice and control over their care. Travel across the Northern Rivers region, from Ballina to Tweed Heads, is required. A company vehicle is provided for business use. Key responsibilities include: Delivering person-centred case management tailored to each client’s needs, goals, and preferences Preparing and reviewing individual budgets in partnership with clients to ensure clarity and alignment with their care objectives Managing a caseload, including regular client assessments and reviews to ensure quality, holistic care Building and maintaining strong, trusting relationships with clients and their families Coordinating services across allied health, nursing, home support, and equipment providers About You To succeed in this role, you’ll bring: Proven experience in aged care case management (HCP/CHSP preferred) Minimum 2 years’ relevant experience in community or aged care coordination Strong planning, coordination, and client advocacy skills—especially within reform environments Nursing qualifications (RN or EN) highly regarded Excellent communication, interpersonal, and problem-solving abilities Capacity to work independently while contributing to a multidisciplinary team Passion for delivering high-quality care with meaningful outcomes Eligibility for a National Police Check and NDIS Worker Screening Check (or willingness to obtain) A valid Australian driver's licence and clean driving history What We Offer Supportive Workplace : Be part of a collaborative, experienced team Recognition & Rewards : Birthday leave, staff recognition initiatives, and a retail discount program Salary : $78,000–$80,000 based on qualifications and experience Legislative 12% superannuation, full-time role with no weekend work Tools of the Trade : Company vehicle for work-related travel and a laptop provided Professional Growth : Access to ongoing training and clinical support Meaningful Work : Be part of a values-led organisation improving lives every day About Right at Home Right at Home Ballina is a trusted local provider of in-home care.
