Case Managers, Home Care Package About us: We are a not-for-profit aged, disability and community care services organisation serving the culturally and linguistically diverse (CALD) communities in metropolitan Sydney. As a Case Manager, you will be employed on a full-time basis at our Sydney metropolitan office within our aged care program. You will be providing support to clients who access our Home Care Package services enabling them to live at home independently with choice, control and with a focus on optimising health and well-being in accordance with the client’s needs, goals and preferences. The role • Undertake person-centred care planning in accordance with ACCA’s model of care, developing care plans, budgets, schedule of services, including identifying client care needs, requirements and setting client service expectations, • Work collaboratively with clients, their carers, family, and/or advocates as well as other Home Care service staff, health professionals and other external stakeholders, to maximise clients' independence and their ability to remain living in the their home and engaging with their community, • Allocate tasks to care workers and ensure compliance with client care plans, • Ensure quality and culturally appropriate service delivery is implemented in a timely manner. Requirements • Qualifications in a related discipline (social sciences, health, psychology, social work, community services), • Minimum 2-3 years’ experience in Aged Care sector - particularly Home Care Packages program and familiar with Aged Care quality standards, • High level of computer literacy, including Microsoft word, excel and PowerPoint, and the ability to learn new software programs, • Demonstrated experience of working with older people, including people from CALD backgrounds, and understanding of issues facing people with chronic and complex needs, • Registered and insured motor vehicle and current unrestricted drivers’ licence, • Ability to speak Cantonese and/or Mandarin. • You are an Australian citizen, permanent resident or hold a visa allowing unrestricted working rights. • Possess or willing to undertake a Nationally Coordinated Criminal History check.
