Key Responsibilities: Manage CHSP (Commonwealth Home Support Program) clients. Act as the first point of contact for potential clients, providing guidance on in-home care options. Coordinate client onboarding, including care planning, agreements, service scheduling, and documentation. Monitor client health outcomes and collaborate with internal teams on care adjustments. Provide support, ensuring care aligns with industry standards and internal policies. Participate in on-call support as rostered and liaise with external providers for holistic care delivery.
