SUPPORT FACILITATOR, THE WAY BACK SUPPORT SERVICE PERMANENT FULL-TIME WIDE BAY Your Next Role! RFQ is seeking a dedicated and values-driven Support Facilitator to join The Way Back Support Service in Maryborough or Bundaberg on a permanent full-time basis. In this role, you'll provide tailored aftercare and empower individuals at high risk of suicide to reconnect with their strengths, community, and purpose. Your week may look like: Integrating and collaborating with existing services to enhance the immediacy of follow-up responses for individuals at high risk of suicide or those who have recently attempted suicide, in line with RFQ’s Models of Practice. Delivering client-centred support through RFQ’s Models of Practice—focused on skills development, capacity building, and strengthening community connections. Conducting suicide risk assessments using the STARS protocol (developed by AISRAP) and participating in collaborative case reviews. Partnering with clients to identify needs, co-design Wellbeing Plans, and assist in accessing community, clinical, and non-clinical supports. Ensuring continuity of care for clients transitioning from hospital to community by liaising effectively with hospital staff and other service providers. Maintaining accurate documentation using both mobile and office-based technologies, and escalating concerns to the Clinical Advisor and Service Manager when needed. Working alongside all RFQ staff to cultivate a culture that values lived experience, fosters genuine and open relationships, and promotes conflict resolution, self-responsibility, mutual respect, and personal growth. Please note, this role requires being on call. You will need: A Diploma or Certificate IV in Community Service (or related field), or significant proven experience supporting individuals living with mental illness or navigating crisis, including suicidal ideation. Understanding of suicidal behaviours, suicidal crisis/ideation and its effects on individuals, families and the whole community. Skills in collaboratively developing and implementing support plans tailored to individual needs and goals. A demonstrated ability to work collaboratively within a team, contributing positively to shared objectives and fostering a values-based environment. Demonstrated effective interpersonal, oral and written communication skills including the ability to advocate for clients and the organisation. Valid driver’s licence and access to a registered roadworthy and insured vehicle (minimum third-party property). Why RFQ?
