The Case Partner role involves supporting older Australians through the Support at Home Program by delivering person-centered care, coordinating services, and building relationships to enhance their quality of life. You will manage a caseload, work closely with families and professionals, and ensure tailored, timely support. Qualifications Qualifications: Certificate IV in Aged Care, Nursing, Allied Health, Social Work, or related field; Degree or equivalent experience preferred. Experience: Experience in case management or service coordination, especially within aged or community care; familiarity with the aged care system is advantageous. Other Requirements: Australian Driver’s License, Police and Working with Children Checks, First Aid and CPR certification, commitment to ongoing professional development.
