About the Role As a Youth Disability Case Manager, you will provide trauma-informed, person-centred support to young people with diverse and complex needs. You’ll be responsible for developing and implementing Individualised Support Plans, coordinating services, and fostering environments that promote emotional, social, physical, and cultural wellbeing. This role intersects youth work and disability support, requiring strong leadership, advocacy, and collaboration with internal teams and external stakeholders. You’ll guide support staff, promote team cohesion, and ensure young people are at the centre of every decision. Key Responsibilities Develop and review Individualised Support Plans in collaboration with young people Promote safety and wellbeing through trauma-informed and culturally safe practices Provide leadership and guidance to support staff and act as an escalation point Coordinate services across health, education, employment, and community engagement Maintain accurate case notes, incident reports, and compliance documentation Advocate for human rights, choice, and control for young people with disability Facilitate structured routines and meaningful activities that support independence Collaborate with NDIS Support Coordinators and multidisciplinary teams Ensure young people have access to essential resources and a homely environment Support operational performance and contribute to NDIS plan development What You’ll Bring Minimum 2 years’ experience in case management within human services Bachelor-level qualification in a relevant discipline Knowledge of trauma-informed practice and disability support frameworks Experience working with young people in out-of-home care or disability contexts Strong communication, leadership, and stakeholder engagement skills Ability to work independently and manage a full workload Understanding of NDIS, safeguarding frameworks, and child safety legislation Valid QLD Driver’s Licence and eligibility for Blue Card, Yellow Card, and relevant screenings Why Join Us?
