The Client Access Disability Services WA is a disability support and health care services company based in Armadale in Western Australia. They are looking for a full time Community Worker at Armadale WA. The Role This is a full-time role based in Armadale, WA, offering an annual salary of $77,000 to $85,000 plus superannuation, depending on experience. The position follows a rotating roster, which includes occasional weekend shifts and reasonable overtime where required to meet client needs. Duties will include: Follow each client's care plan and help deliver the right support, education, or training they need in their daily lives Keep track of how clients are progressing by taking notes after each shift and sharing updates with the wider support team Join meetings when needed, especially when we're reviewing a client's needs or discussing available support options like housing, health services, or education Provide hands-on support in clients' homes or in shared community housing, including offering care and guidance for people living with disabilities Help clients deal with social, emotional, or financial challenges so they can live more independently and confidently Assist in preparing reports or documents required for funding bodies, government agencies, or other organisations we work with Support clients during community activities such as going shopping, attending appointments, joining social outings, or achieving personal goals Accompany clients to medical or therapy appointments, hospital visits, or respite services to make sure they get the care they need Help maintain a safe and clean living space by doing light tasks like changing bed sheets or tidying up when needed-especially when it supports the client's health and comfort Requirements A Diploma in Community Services, Certificate 3 or Cert 4, or a related qualification is preferred Minimum 2 years' experience in community or aged care settings Minimum 2 years of relevant experience in a professional setting Minimum 2 years of relevant experience in community or aged care Minimum 2 years of experience dealing with mental and emotional disturbances and developmental disability Minimum 2 years of experience working with government agencies, non-profits, and other service providers Confident working with children and vulnerable groups in a respectful, supportive manner Must hold a valid driver's licence and be willing to travel between client sites if required Must have (or be able to obtain) a current Working with Children Check and a clear National Police Check Must hold, or be willing to obtain, a valid First Aid (HLTAID011) and CPR (HLTAID009) certificate Strong communication and interpersonal skills, with a compassionate and professional approach
