About us: Aged Care Assessment (ACA) is a multi-disciplinary team of nursing and allied health clinicians whose main function is to assess frail clients 65 years and over and Indigenous clients 50 years and over for: Permanent Residential Aged Care Respite Residential Aged Care Home Care Package Program in the community Transition Care Program (12-week community rehabilitation program - therapy and case management intervention from direct hospital discharge - with 24 hours) Short Term Restorative Care (8-week community rehabilitation program - community therapy intervention for aged functional decline) About the role: The Social Worker for the Aged Care Assessment (ACA) is the Senior in the team who provides leadership and training support to the Clinicians within ACA. This is a leadership position. The Social Worker Senior works within the Aged Care Assessment (ACA) completing ACA assessments in hospital or community settings according to Commonwealth guidelines and the Aged Care Act 1997, bringing a Social Work focus to the multi-disciplinary team and to client care, and referring frail aged clients to appropriate service provision. The Social Worker Senior - ACA provides comprehensive assessment and care planning which focuses on client health and health related needs following a wellness and reablement model of practice. About you: The successful applicant must hold a tertiary qualification, or equivalent in Social Work. Previous Aged Care Assessment experience would be an advantage.
