The Care Finder program was announced in the 2021-22 Commonwealth Budget as part of the Connecting Senior Australians to Aged Care Services measure. Care Finders will: Provide specialist and intensive assistance help people to understand and access aged care and connect with other relevant supports in the community; and Specifically target people who have one or more reason for requiring intensive support to interact with My Aged Care, access aged care services and/or access other relevant supports in the Mitchell and Whittlesea shire Provide assertive outreach while building engagement and rapport with potential clients Guide clients through the assessment process and assist in obtaining Aged Care supports and services needed Skills And Experiences Bring your great, everyday! To be considered for this rewarding role you will have: Tertiary qualification in Human Services/Social Work or related profession and or previous experience in needs assessment, care planning and/or case management Extensive knowledge of the aged care, health and community services system and regulatory environment Experience in working with older people and their families/carers from a range of backgrounds, and an understanding of the barriers they may experience in accessing aged care services The ability to work collaboratively and autonomously and build effective relationships Demonstrated experience in community networking and working collaboratively with external service providers to plan and provide consistent, responsive, and appropriate client focused services.
