About ECHO ECHO Community Services is a registered charity providing care to the elderly to help them live in their own home with dignity while still maintaining their independence. We provide in-home services 7 days per week for Home Care Package and 5 days per week for CHSP. Our goal is to provide services at the same time, on the same day and with the same team of Support Workers which means that you know the people who are coming and the time they will arrive. Having the same support workers providing services, in particular, medication prompts and personal care, maintains the person’s dignity and well-being. You know and can trust the people attending. There is continuity of care for people who receive our services. Description We are currently seeking an experienced and motivated Occupational Therapist to join our Home Care Team on a part-time basis (minimum 3 days per week). Reporting to the General Manager Operations, you will play a vital role in supporting clients to live safely and confidently in their own homes through tailored occupational therapy services. Key responsibilities: Conduct assessments and prescribe Assistive Technology and Home Modifications to support independent living Supporting Home Care staff to undertake activities and purchasing of aids and equipment to meet client’s needs including mentorship and training Providing support to integrate new programs Provide clinical support and advice to address client-specific challenges Participate in multidisciplinary care planning and case conferences Liaise with external agencies and contractors as appropriate to support clients. Hours: Minimum 3 days per week, from 8:00 am to 4:00 pm. Flexible working days available. Skills And Experiences Qualifications: Current registration as an Occupational Therapist with AHPRA (essential) Valid Australian driver’s licence and willingness to travel Essential: Significant experience in prescribing assistive technology and home modifications Demonstrated ability to conduct functional assessments and develop person-centred therapy plans Knowledge of aged care quality standards, funding models (HCP, CHSP, TCP), and clinical documentation Competencies: Above average communication, interpersonal and time management skills Ability to work independently and as part of a multidisciplinary team Demonstrated positive attitude towards change management and new initiatives Exceptional client/customer service skills Strong attention to detail and accuracy in documentation and assessments.
