Key Responsibilities : Client management and oversee client care assessments Coordinate and monitor delivery of services under Home Care Packages (HCP)/Support at Home Support intake, onboarding and service transitions for new clients Provide clinical oversight and guidance to support workers Supervision and management of support workers Liaise with families, GPs, allied health professionals and other stakeholders Ensure compliance and quality as per standards Optimise budgets and resources Onboarding and training of support workers Community engagement and nursing visits To be successful : Current AHPRA registration and clinical assessment experience Experience working in Home/Community Care as a Case Coordinator or Care Manager Experience and success in working in a customer focused service business Demonstrated experience in managing and coordinating Home Care Packages Excellent engagement and communication skills Amazing customer service, friendly approach, compassion and values focused Current driver's licence and willingness to use own car with kilometre reimbursement National Police Clearance within the last 3 months What you can expect in return : $100K Superannuation Laptop Phone Company Vehicle and Fuel Card and On-site parking available Permanent Full Time - Monday to Friday Build lasting relationships with clients and represent the company Amazing organisation with leaders who value your contribution!
