We are a dedicated, innovative not-for-profit community organisation with over three decades of experience supporting elderly clients to connect and thrive within our Community Day Centre. We are currently seeking a Community Service Coordinator to join our team on a part time basis. Woking as part of a friendly and close-knit team, your focus will be: Coordination and scheduling of services and appointments for CHSP and Allied Health clients. Serve as a contact for our community service clients, Independent Living Unit Residents and Allied Health clients. Responding and following up queries as requested. Participate actively in the research and development of the social support program. Focusing on social inclusion and health and wellbeing. The ability to build rapport and strong working relationships with customers by demonstrating accountability, adaptability, professionalism and engagement Manage the onboarding process of new clients ensuring documentation is compliant. Work with your team to create a friendly welcoming environment for all clients Help coordinate volunteer by giving them direction with daily tasks within the program and act as a ‘go to’ for any volunteer concerns. Assist clients to achieve goals that will help with independence and health and wellbeing. Assist Community Service Manager as required Always maintain client privacy. About you You are an enthusiastic, motivated, self-starter who is eager to develop a career within our Community Centre. To be considered for this role, you will need: Previous Experience (2 years) of similar role in a Community Centre/Aged Care setting. Certificate IV or Diploma in Community Services, Aged Care or equivalent Current First Aid Certificate Demonstrated initiative and ability to be self-motivated, work without supervision and promote a positive lifestyle environment Strong customer service focus Excellent communication and interpersonal skills Ability to work independently and collaboratively as part of a multidisciplinary team. Well-developed prioritisation and organisational skills with the ability to meet deadlines Sound computer skills. Strong planning and time management skills with the ability to prioritise. Current Satisfactory Police Check Victorian Driver License
