This role is responsible for directing and supervising clinical operations and ensuring the achievement of agency goals and objectives related to patient satisfaction, clinical outcomes, productivity, and utilization. A Home Health Registered Nurse (RN) Care Manager in Maitland, FL, coordinates and directs a patient's care within the home setting. They independently manage a caseload of home health patients, requiring advanced assessment, teaching, and decision-making skills. They assess patient needs, develop and implement care plans, educate patients and their families, and monitor the effectiveness of these plans.
