Your role includes: Assisting clients with maintaining their rosters of support to meet their individual goals. Manage client information to ensure all relevant records and correspondence are current and provide an accurate reflection of service provision. Coordinating meet and greets for choice and control. Our Applicant will have: An engaging Person-centred approach to Support Services Exceptional communication and relationship building skills A genuine desire to make a difference in the lives of people living with disability Have an exceptional understanding of support services, geographical locations, and essential services.
