As a Care Coordinator with our Salvos Home Care team, you will benefit from: Part time role with generous salary and flexible work arrangements Generous Salary Packaging benefits Excellent Support and education opportunities Award winning not for profit aged care provider ABOUT THE ROLE The Care Coordinator is primarily responsible for the ongoing coordination and management of a comprehensive range of services and supports provided to an allocated group of Home Care Package or CHSP clients. This will be achieved through building partnerships with clients and their families, and the Salvos Home Care team. The Care Coordinator is well supported by the Salvos Home Care Operations Manager, a stable team, and the broader Aged Care Support Services (Head Office). Undertakes initial assessment and on-boarding for new clients and maintains ongoing assessment and exemplary relationship with clients and their representatives Develops, implements and monitors consumer directed care plans in collaboration with clients Provides consumer directed case management services to clients acting as the main point of contact between clients, their informal carers, Home Care Workers and external service providers Establishes and maintains positive working relationships with all members of the Salvos Home Care team and with the broader Aged Care Support Services team ABOUT YOU An understanding, empathy, appreciation and the ability to promote the values and ethos of The Salvation Army. Current unrestricted registration with Australian Health Practitioners Regulation Agency is highly regarded. Motivation to work with older people living in the community and an understanding of consumer directed care and enablement approaches Proven leadership experience in overseeing, coordinating and implementing consumer directed care services within a Home Care setting Excellent oral and written interpersonal skills, strong customer service focus and ability to negotiate and influence others Current drivers licence, good driving record and willingness to travel for work.
