About the role: The position of Physiotherapist assesses, plans and implements physiotherapy interventions to improve mobility, strength and functional abilities of our participants. You will conduct assessments, develop customised treatment plans, and provide hands on therapy. By supporting our participants who receive funded aged care to enable them to live in the community; you will collaborate with other healthcare professionals and families to support the individual's well-being. Reporting to the Allied Health Manager you will collaborate on a daily basis with our Care Partners, Schedulers, Administration team, our Care Workers, other Allied Health team members and our Clinical team. Key responsibilities include but are not limited to; Conduct assessments to determine the participants needs and capabilities Develop individual treatment plans that address specific needs and goals of each participant Provide hands on physiotherapy interventions, including exercises, manual therapy techniques and other modalities to improve strength and balance Educate our participants and their families about the importance of physical activities and safe movement techniques Maintain accurate and detailed reports of assessments, plans and progress notes Provide ongoing support and advice Liaise with care staff delivering direct services and identify changes in support needs of participants This is a 12-month Full-time Fixed Term Contract. To be successful in this role you will bring: Tertiary qualifications in physiotherapy and current full registration with AHPRA as a Physiotherapist An understanding and appreciation of contemporary Physiotherapy practice and issues facing in home care of individuals Well-developed Physiotherapy clinical skills with substantial experience with treating older individuals Minimum 3 years post graduate experience WA Drivers License Strong clinical reasoning and communication Skills in assessment, equipment prescription and problem-solving Ability to work autonomously and manage time effectively Experience or knowledge in aged care, community services, My Aged Care, CHSP or Support at Home will be highly regarded What Southcare can offer you! 12% superannuation competitive Salary Packaging benefits of up to $15,900 per annum tax free (more after tax $ in your pocket!)
