We are seeking an experienced and motivated Chronic Disease Management Nurse or Nurse Practitioner to join our growing team on either a full-time or part-time basis . School hours will be considered for the right applicant. This position is an excellent opportunity for an experienced Registered Nurse or Nurse Practitioner who is passionate about preventative healthcare and chronic disease management. You will enjoy building strong, lasting relationships with patients while empowering them to achieve better health outcomes and making a meaningful difference in the management of chronic and complex health conditions. About the Role Working closely with our GPs and multidisciplinary team, you will lead and coordinate chronic disease management services while providing holistic, patient-centred care. Key Responsibilities Coordinate and deliver comprehensive Chronic Disease Management services, including GP Chronic Condition Management Plans, reviews, and health assessments. Conduct clinical assessments and develop individualised management plans in collaboration with GPs. Provide patient education, self-management support, and preventative health advice. Monitor patient progress and coordinate ongoing follow-up care. Perform health assessments, care planning, recalls, and quality improvement activities. Administer immunisations, medications, wound care, ECGs, spirometry, and other clinical nursing procedures where required. Support GPs with clinical care and minor procedures as needed. Maintain accurate clinical documentation using Best Practice software. Liaise with allied health providers, specialists, hospitals, and community services to coordinate patient care. Participate in quality improvement initiatives and contribute to achieving accreditation standards. About You To be successful in this role you will have: Current registration with AHPRA as a Registered Nurse or Nurse Practitioner. Demonstrated experience in Chronic Disease Management within general practice or primary healthcare. Sound knowledge of preventative health and evidence-based chronic disease management. Excellent clinical assessment, communication, and organisational skills. Experience preparing and coordinating GP Chronic Condition Management Plans and Health Assessments (highly desirable). Immunisation certification (desirable). Experience using Best Practice software (desirable). A proactive approach, excellent time management, and a commitment to exceptional patient care.
