About the Role As our Community Physiotherapist, you'll provide evidence-based physiotherapy services to older people in their own homes, helping them remain safe, active and independent. You'll work closely with Registered Nurses, Enrolled Nurses, Community Care Workers and other health professionals to deliver coordinated, holistic care. You'll primarily support clients across Canberra's northside and surrounding NSW communities, enjoying the variety, autonomy and flexibility that community-based practice offers. What You’ll Do Deliver person-centred, evidence-based physiotherapy in clients' homes and community settings Conduct comprehensive assessments and develop tailored rehabilitation, mobility, strength, balance and exercise programs Support clients to achieve their goals and maximise independence Recommend and prescribe appropriate mobility aids, equipment and home safety strategies Collaborate closely with clients, families and the multidisciplinary team to deliver coordinated care Participate in multidisciplinary High Risk meetings and contribute to care planning Assist in developing our future community clinic and Allied Health services Identify opportunities to improve client outcomes and enhance service delivery Maintain accurate clinical documentation in line with professional standards About You You’ll be someone who genuinely enjoys working with older people, values relationship-based care and is motivated to make a lasting difference in the community. Essential Degree qualification in Physiotherapy Current AHPRA registration as a Physiotherapist Current Australian working rights Current Driver’s Licence and willingness to travel within the community Current First Aid and CPR certification National Police Check (or willingness to obtain) Excellent communication, interpersonal and organisational skills Ability to work independently while contributing to a multidisciplinary team Desirable Experience in community aged care, home care, rehabilitation or disability services Knowledge of Home Care Packages, Support at Home and/or NDIS funding Experience with falls prevention, exercise prescription and wellness programs Experience with Assistive Technology Strong relationship-building skills with clients, families and stakeholders A proactive, solution-focused approach to client care Passion for innovation, continuous improvement and service development Commitment to ongoing professional development and evidence-based practice About Us At Right at Home, we believe everyone deserves the right care, right at home.
