About the role CareWA Services is seeking a compassionate and dedicated Case Manager - NDIS Sector to join our growing team in Myaree, WA . This is a full-time position that plays a vital role in supporting individuals with disabilities to achieve their goals and maximise their independence through the National Disability Insurance Scheme (NDIS). As a Case Manager, you will be at the forefront of delivering person-centred support, building meaningful relationships with participants, and coordinating comprehensive care plans that enable positive life outcomes. Key responsibilities Conduct comprehensive assessments of participant needs, goals, and aspirations to develop individualised support plans aligned with NDIS objectives Coordinate and manage support services, ensuring participants receive appropriate and timely interventions Establish and maintain strong, trusting relationships with participants and their families or carers Monitor participant progress against agreed goals and adjust support strategies as required Liaise with external service providers, medical professionals, and community organisations to ensure holistic support delivery Prepare and maintain detailed case notes and documentation in compliance with NDIS requirements and organisational policies Advocate for participant rights and ensure their voice is central to all decision-making processes Identify and address barriers to participation, inclusion, and access to community resources Participate in team meetings and contribute to continuous service improvement initiatives Support participants to build their capacity and independence over time What we're looking for Tertiary qualification in Social Work, Disability Services, Aged Care, Nursing, Psychology, or a related discipline (Not essential but preferred) Demonstrated experience working in the disability sector, NDIS environment, or community services Strong understanding of the NDIS framework, participant planning processes, and support coordination principles Excellent interpersonal and communication skills with the ability to engage sensitively with participants from diverse backgrounds Proven ability to develop and maintain professional relationships with participants, families, and external stakeholders Strong organisational and time management skills with the ability to manage multiple cases effectively Proficiency in using IT systems and electronic case management software Current Working with Children Check (or willingness to obtain) and a valid Australian driver's licence Demonstrated commitment to person-centred practice and social justice principles Ability to work both independently and collaboratively within a team environment Preferred: Experience with outcome measurement and data collection in disability support services What we offer CareWA Services is committed to supporting the wellbeing and professional development of our team members.
