We deliver person-centred care with compassion, professionalism, and respect. About the Role: We are seeking a compassionate and dedicated Community Worker to join our growing team in Sydney. You will provide high-quality support to clients in their homes and within the community, helping them maintain their independence, wellbeing, and quality of life. Key Responsibilities: - Provide person-centred care and support to clients based on individual care plans. - Assist clients with daily living activities, including personal care, meal preparation, and household tasks. - Support clients to attend appointments, social activities, and community programs. - Encourage clients to develop independent living skills and achieve their personal goals. - Maintain accurate records and complete required documentation. - Communicate effectively with clients, families, and healthcare professionals. - Follow workplace health and safety policies and maintain client confidentiality. Skills and Qualifications: - Minimum 1 year of experience in a Community Worker, Disability Support Worker, Aged Care, or similar support role. - Strong communication and interpersonal skills. - Compassionate, reliable, and committed to delivering high-quality care. - Ability to work independently and as part of a team. - Current First Aid and CPR certification (preferred). - Valid driver's licence and access to a reliable vehicle (preferred). - Relevant qualification in Community Services, Individual Support, Disability, Aged Care, or a related field is desirable.
