Lead your team in making a real difference supporting older people to live well at home. We’re now seeking a Care Partner Lead to join our Community Services team in the Limestone Coast. In this full-time permanent role, you’ll lead the team in coordinating and delivering personalised, high-quality care and services that help older people achieve their goals and maintain independence at home. Responsibilities: Providing Leadership, Mentoring and Support to Care Partners Coordinating and managing individual care plans and service delivery Monitoring budgets and outcomes to ensure high-quality, person-centred care Building supportive relationships with clients, families, and community networks Supporting Home Support Workers and contributing to a positive team culture Ensuring compliance with Support at Home program guidelines and Resthaven’s policies Essential Criteria: Tertiary qualification in Nursing, Social Work, Allied Health or another relevant health-related discipline (Essential) Current registration with the relevant professional governing body (e.g. AHPRA) (Essential) Program Coordination, Team Leading and case management experience. Experience leading teams to meet budget KPIs whilst delivering a high quality of care Empathy and understanding of issues for older people, their families, and carers A commitment to supporting older people to remain at home in the community An ability to work with individuals’ to identify their needs, develop personal goals and develop responsive packages of services Knowledge and experience of quality systems for evidence-based care Computer literacy SA Driver's Licence Why Resthaven?
