About us: Bannister In Home Care delivers in-home support services to clients with varied needs due to ageing, acquired or inherited disability or injury. We provide services on behalf of the Australian Government and strictly comply with legislated requirements and standards. We are seeking a Registered Nurse (Aged Care) to join our team. Key Responsibilities Client Assessment: Interviewing new clients (Support at Home, NDIS, and Private) to assess their needs and services required. Developing, monitoring, and reviewing Person Centred Care Plans. Service Coordination: Collaborate with medical practitioners, allied health professionals, Australian Government Organisations and care staff to deliver high-quality client care. Medication Administration and Clinical Care: Administer medications, treatments and other clinical interventions, and monitor residents' responses to care. Independence & Reablement: Promote residents' health, wellbeing and independence through health education and preventative care initiatives. Resident and Family Education: Provide information, guidance and support to residents and their families regarding health conditions, treatment plans and care needs. Health Promotion and Disease Prevention: Participate in health promotion activities and contribute to the prevention and management of illness. Health Monitoring and Clinical Reporting: Monitor, document and report changes in residents' health conditions and escalate concerns where appropriate. Compliance: Ensure compliance with aged care standards, infection control requirements and organisational policies and procedures. Clinical Leadership and Supervision: Provide clinical guidance, supervision and support to Enrolled Nurses, Personal Care Workers and other care staff to ensure the delivery of safe and high-quality care. Experience working in the Aged Care or Disability sector Experience in team leadership/management positions Working Knowledge of the Regulations, standards, frameworks and codes Current Police Check & Current First aid Certificate Up to date immunisation Work Environment & Requirements Registered Nurses (Aged Care) are expected to travel between client homes and meeting sites. Must be able to drive anywhere in the Sydney and Greater Sydney region.
