About the role: This role is responsible for coordinating the NDIS planning process to ensure the people we support receive sufficient funding to meet their needs and achieve their goals. The role supports new and existing transitions of clients and services, ensures relevant agreements are accurate, and all services are billed. Further, the role contributes to the implementation and embedding of existing client related frameworks and ways of working to ensure sites and teams are set up for success. Role Responsibilities: Assist the people we support with funding renewal, plan variations and re-assessments, for both NDIS and DSOA participants (where relavent), utilising the various systems and processes including NDIS funding Portal for setting up and managing service bookings. Maintain the centralised data set of people we support, existing funding and service types, with service agreement dates. Identify and escalate any issues relating to people we support funding. Consult with the Service Manager to support the funding planning process for our participants, to receive the necessary level of funding, to be well supported and meet their goals. Collaborate with Service Manager and Support Coordinators to gather sufficient evidence for funding applications. Draft and finalise the Statement of Service or ISPs for DSOA participants and prepare the Service agreement for each participant and forward on to relevant parties for final approval and distribution to plan nominees. Maintain and update all relevant agreements, as required, including the completion and filing of the signed documentation for all the participants, their funding, funding renewal dates, and any expiring agreements. Work collaboratively with Community Engagement Lead to ensure all new prospects are engaged, coordinate the transition process with the front-line management team, and communicate potential new vacancies as they arise. To be successful in this role, you will need: Demonstrated experience in interpreting the NDIS price guide, statutory requirements, planning decisions, and reasonable and necessary scope of plan funding. Sound knowledge and understanding of legislation and regulations governing the Disability sector and related issues. Demonstrated experience in a client relationship role where developing, building, and maintaining relationships. Demonstrated ability to use Proda portal, Visicase and any other relevant NDIS systems. Understanding of the Roster of Care process. Understanding of the client intake process. Strong interpersonal and relationship-building skills. Good conceptual, analytical, and problem-solving skills. Advanced interpersonal communication skills including negotiation and the preparation of written and verbal reports. Demonstrated excellence in customer service. An ability to prioritise work and meet conflicting deadlines and excellent time management skills Experience in a fast-paced environment, managing multiple priorities and deadlines across a range of internal and external stakeholders.
