This is a unique opportunity to support clients across Metro Melbourne and Regional Victoria during important transition periods — including hospital discharges, emergency placements, and provider changes. What You’ll Be Doing Deliver person-centred care Step into urgent or complex support situations Assist with new client transitions and stabilising care arrangements Support clients with personal care, community access, and daily living Provide high intensity and complex care support (where trained) Maintain clear communication and accurate documentation What We’re Looking For Certificate IV in Disability, Aged Care, Community Services, or similar Experience supporting clients with complex needs and behaviours Current First Aid, CPR, NDIS Worker Screening Check, and Working with Children Check Victorian driver’s licence and fully insured vehicle Strong communication and problem-solving skills Flexibility to travel across Victoria, including occasional overnight stays Nursing backgrounds highly regarded
