We are seeking a Part-Time Registered Nurse role for Chronic Disease Management. Key Responsibilities Work collaboratively with General Practitioners to improve patient outcomes through effective Chronic Disease Management Identify eligible patients who would benefit from GP Management Plans and Team Care Arrangements Conduct comprehensive health assessments to identify health risks and promote disease prevention Deliver high-quality clinical care including triage, immunisations, injections, wound management, ECGs, and collection of pathology samples Complete pre-employment medical assessments in line with organisational requirements Ensure all patients receive a high standard of customer service through effective communication, professional presentation, and clinical expertise Respect and uphold patient rights at all times, ensuring culturally sensitive and person-centred care Provide appropriate referrals to allied health professionals and specialists as required (e.g. GP, optometrist, diabetes educator) Manage recalls, reminders, and administrative clinical tasks, ensuring accurate and timely documentation Maintain clinical equipment to ensure safe and accurate use, including calibration and monitoring expiry dates Adhere to privacy, confidentiality, and organisational policies and procedures at all times Assist doctors with minor procedures as require Chronic Disease Management & Health Assessment Focus Preparation and coordination of Chronic Disease Management documentation, including GP Chronic Condition Management Plans and reviews Preparation of referral forms for allied health services for patients with chronic and complex care needs Conduct and support structured health assessments, including: Time-tiered health assessments, Aboriginal and Torres Strait Islander health assessments, Heart health assessments. Act as a patient advocate in the ongoing management of chronic disease conditions Key Requirements Current unrestricted registration with AHPRA as a Registered Nurse Experience in general practice or primary health care (highly regarded) Strong experience in chronic disease management and health assessments Ability to work autonomously and as part of a multidisciplinary team Strong organisational and time management skills in a busy clinical environment High level of computer literacy, preferably with experience in Best Practice software Commitment to delivering patient-centred care and maintaining ethical and professional standards
